Educational Psychological Assessments
A private referral service is now being offered to members of the general public. We provide a service for individuals aged five years and upwards with specific learning difficulties such as dyslexia or dyspraxia, as well as associated difficulties such as Asperger’s syndrome and attention deficit hyperactivity disorder or ADHD.
An educational psychological assessment costs €550.
Individuals may be referred to the National Learning Network Assessment Service by professionals, family members or through self-referral. In all cases, the Assessment Service follows the highest professional standards and places the individual at the centre of the service.
To make a referral, please download a copy of the relevant questionnaire/s by selecting from the list below. Alternatively, you can contact the Assessment Service directly on 01 885 1386 and the questionnaires can be posted to you.
If the individual is less than 12 years of age, please complete the parent and teacher questionnaires, along with the consent form.
If the individual is aged between 12 and 16 years, please complete the adolescent, parent, and teacher questionnaires, along with the consent form.
If the individual is over 16 years of age, please complete the adult questionnaire and the tutor/trainer/employer questionnaire (where possible) in addition to the consent form.

Please note that all returned questionnaires must be accompanied by:
1. the completed consent forms;
2. a cheque/cash deposit for €100 - which will be deducted from the overall fee of €550 at the time of the assessment - and
3. copies of previous assessment reports
